Hematuria is transient, physiological, and usually not accompanied by pain. Red urine and pain are a sign of serious pathology.
Factors that cause a combination of symptoms - hematuria and pain - may be:
- Ureteral occlusion, absence of urine passage (calculi, blood clots block the urethra). In nephrolithiasis, pain precedes the appearance of red urine.
- Obstruction of the urethra as a result of tumor growth (papillary tumor of the bladder). The beginning of the process is painless, urine is red and pain is a sign of tumor growth in the bladder tissue.
- Bladder tamponade due to trauma, tumor, prostatic hyperplasia, spontaneous rupture of the prostate capsule.
- Pyelonephritis in the acute stage.
- Acute prostatitis (abscess).
- Rupture of the veins of the bladder (varicose veins).
- Uterine fibroids with complications.
- Polycystic ovary syndrome.
- Terminal stage uterine tumor.
- Urolithiasis.
- Glomerulonephritis.
- Late stage renal cell carcinoma.
Almost all of the above reasons fall into the category of serious pathologies requiring urgent intervention, in 90% of cases using surgical methods.

Red urine and abdominal pain
Discomfort in the abdomen combined with urine of an atypical color is a symptom that should not be ignored. Most often, red urine and abdominal pain are caused by serious diseases that require immediate medical intervention. Hematuria is a clinical designation for the presence of blood cells in the urine, which in itself is an indicator of the development of the pathological process. Red urine is not considered an independent symptom; in combination with painful manifestations, hematuria may indicate the following diseases:
- Red urine and abdominal pain localized in the upper abdominal area or in the side may indicate an inflammatory process in the upper sector of the urinary system (tract).
- Pain irradiation from the abdominal area down to the genitourinary organs (testicles or vagina) in combination with red urine may be a sign of ureteral obstruction by stones or blood clots. Urolithiasis often causes an increase in body temperature, blood pressure surges. In addition, pain of this nature is considered one of the symptoms of sickle cell hemoglobinopathy (anemia), diabetic necrotic papillitis, tuberculosis of the kidney, and terminal cancer.
- Thrombosis, embolia of the renal arteries also manifests itself as hematuria and abdominal pain, most often in the iliac region.
- Red urine and abdominal pain in the lateral areas (simultaneously or only on the left, right) are a possible sign of inflammation of the ureter.
Pain is considered one of the leading symptoms in urological practice, it is especially dangerous when it manifests itself in the abdominal area. Blood in the urine, urine of an atypical color in combination with pain is an indisputable reason for immediate qualified medical care.
