What causes a rupture of the collateral ligaments of the first metacarpophalangeal joint?

Rupture of the lateral ligaments of the first metacarpophalangeal joint usually occurs in athletes during unsuccessful attempts to perform exercises on gymnastic apparatus and is the result of forced excessive abduction of the first finger. The ligament located on the side facing the second finger is most often damaged.

Symptoms of rupture of the lateral ligaments of the first metacarpophalangeal joint

The patient complains of pain and dysfunction of the first finger following the injury.

The anamnesis shows a characteristic injury.

Inspection and physical examination

The finger and hand in the area of the eminence of the first finger are sharply swollen. Active and passive movements in the metacarpophalangeal joint are limited due to pain and swelling of the surrounding tissues. After anesthesia or several days after the injury, excessive deviation and dislocation of the first finger can be detected. Opposition of the finger is impossible.

Laboratory and instrumental studies

On radiographs, one can see a rupture of the cortical plate (if the ligament is torn) or a subluxation of the finger.

Conservative treatment of rupture of the lateral ligaments of the first metacarpophalangeal joint

Conservative treatment of rupture of the lateral ligaments of the first metacarpophalangeal joint is prescribed for fresh injuries. It consists of fixing the first finger with a plaster cast in a position of opposition to the second finger for a period of 3 weeks. UHF is prescribed through a cast from the 3rd day, after the end of immobilization - rehabilitation treatment.

Surgical treatment of rupture of the lateral ligaments of the first metacarpophalangeal joint

If the ligament is not restored, surgical intervention is used.
