A disease associated with metabolic disorders is called gout. It is characterized by the deposition of uric acid salts in the joints. Today, gout on the legs is a rare disease, it occurs in three people out of a thousand. It mainly affects men over 40 years of age. Women develop gout during menopause.
Causes gout on the feet
The main cause of the disease is an excessive level of uric acid in the blood. The course of the disease is characterized by significant deposition of urate crystals in the joints. They can accumulate in organs and other body systems. The causes of gout on the legs are due to metabolic disorders. As a result, small particles of sodium urate are freely deposited in the joints, leading to its partial or complete destruction.
Excessive amounts of uric acid occur for two reasons. The first is that healthy kidneys are unable to cope with the removal of large amounts of this "material". The second is that acid is produced normally, but the kidneys are unable to remove it.
Every year gout affects more people, despite its uncommonness. Doctors associate this phenomenon with the consumption of food rich in purines. Alcoholic beverages are also included here. Previously, it was not easy to get gout, there were practically no products that provoked its development.

Pathogenesis
The disease is based on an elevated level of uric acid contained in the blood. This is the pathogenesis of gout. Today, this symptom is comparable to a disease called hyperuricemia. Changes in the level of uric acid occur due to damage to the kidneys and blood. This process can be caused by excessive physical activity and the consumption of large amounts of fatty and unhealthy food.
There are three main elements that provoke the development of gout. These include: accumulation of a significant volume of uric acid compounds in the body, their deposition in organs and tissues, development of acute inflammatory attacks in the affected areas. Finally, this includes the formation of so-called gouty granulomas and tophi (usually they occur in the location of joints).
Symptoms gout on the feet
The first symptoms are always short-term. It all starts suddenly, most often at night. The inflammation affects the big toes. Less often, the lesion is observed on the hands, heels, knees and wrist joints. The main symptom of gout on the legs is sharp pain. A person simply does not know what to do with it and literally "climbs the walls". The affected joint is characterized by redness and swelling. The finger is very hot to the touch. A light touch to it causes a lot of unpleasant sensations, including severe pain.
Any movement brings unbearable pain, it continues for 4 days. Then everything goes away on its own and the person continues to live the same life. After a certain time, the attack is repeated again. It is characterized by sudden development and severe course. The situation will repeat until the person takes measures. After all, over time, the attacks will be longer and more painful. As a result, there comes a time when pain and inflammation are present constantly, and not periodically. Thus causing the development of chronic gouty arthritis.
Sodium urate crystals can be deposited under the skin, causing hard nodules to form. They are filled with a mushy mass. The nodules are called tophi. Their main location is the auricles near the crystals. After the tophi breaks through, the wound heals on its own.
In addition to all the above symptoms, gout is characterized by urate deposits in the kidneys. This leads to the development of urolithiasis, and sometimes pyelonephritis. Gout in women is much easier, it is characterized by the absence of acute attacks. Its main location is the ankle joint.
First signs
The main symptom of the disease is acute pain in the joint of the big toe. The attack begins both late at night and in the morning. The first sign is a pressing pain that causes a lot of inconvenience to a person. The joint begins to swell, the temperature at the site of the lesion increases significantly. The skin is characterized by pronounced redness and may become shiny.
During the day, the victim feels better, but by night the situation worsens. The duration of the attack does not exceed 3 days. In some cases, it can last a week. With a repeated attack, the situation worsens, the time of suffering increases. If no measures are taken, partial destruction of the joint is possible.
Another of the earliest signs of gout is the appearance of a growth at the site of the affected joint. This indicates a sharp increase in the level of uric acid. The growth grows and eventually bursts, the wound heals on its own.
Gout of the big toe
This disease is most often localized on the big toes, characterized by a chronic course. The cause of the disease is excessive deposits of uric acid and salts. Gout on the foot with damage to the big toe never goes away without a trace. Over time, it leads to discomfort and severe pain.
The disease has another name - gouty arthritis. It affects males, or rather, they suffer more often. In women, gout is extremely rare during menopause. The risk group includes people with excess weight and metabolic disorders.
Often the disease occurs due to genetic predisposition. In this case, its development is provoked by several main factors. These include: consumption of high-calorie food, excessive alcohol consumption, poor nutrition and excessive physical activity. Provoking factors are: excess weight, passion for coffee drinks and chronic kidney disease.
Forms
All existing diseases are entered into a single database. Each disease is marked with its own code, which is international. According to the ICD 10 code, gout is recorded under the number (M10).
- M10.0 Idiopathic gout. This category includes gouty bursitis, primary gout, and gouty nodules.
- M10.1 Lead gout

Diagnostics gout on the feet
The diagnosis can be made based on epidemiological diagnostic criteria. Chemical testing allows determining urate and uric acid deposits in the joints. Diagnosis of gout on the legs includes examining the patient and collecting anamnesis about his condition. It is necessary to study the location of the swelling, assess the degree of pain syndrome and the duration of attacks. Then the tophi, their appearance and objective signs are studied.
X-ray examination allows to confirm the presence of inflammatory processes in the joints. It is often used when examining the chronic form of the disease. There are certain criteria, the coincidence of which indicates the presence of the disease. The first of them is the detection of urates in the synovial fluid. The second criterion is the formation of tophi. The third is an acute attack of arthritis, the fourth is severe inflammation, the fifth is the monoarticular nature of arthritis. The sixth criterion is severe pain and swelling. The detection of several symptoms in a person allows a preliminary diagnosis to be made.
Tests
If gout is suspected, laboratory tests should be performed. The first step is to take a general blood test. It can be used to determine the quantitative content of uric acid and neutrophils. The level of these components changes significantly during the progression of the disease.
Urine analysis allows you to determine the amount of protein. Together, these two studies help the doctor get a general picture of what is happening. The specialist assesses the patient's condition, the degree of danger and, based on this, prescribes effective therapeutic measures.
Tests alone may not be enough, in which case additional diagnostic methods are used. They will help to complete the picture of what is happening, make a correct diagnosis and begin high-quality treatment.
Instrumental diagnostics
These diagnostic methods are applicable to almost all patients. They allow to complete the overall picture of what is happening. In case of gout, instrumental diagnostics are of little information, but only in the early stages. Despite this, these research methods allow to distinguish gout from other rheumatological pathologies. Widely used: ultrasound, scintigraphy with technetium pyrophosphate, CT and radiography.
- Ultrasound. Changes can be noticed only during the period of exacerbation of the disease. It is advisable to conduct an ultrasound on the 4th day after the onset of an attack. Deviations from this figure are unacceptable. Thus, on the 7-12th day, the study will not be able to show anything. In the chronic form of the disease, ultrasound will allow you to determine the deformation of the articular surfaces and the presence of tophi.
- Technetium pyrophosphate scintigraphy. This study allows for an accurate diagnosis. It is based on the introduction of a special substance into the blood that can accumulate in places where urates are deposited. Additional scanning will help determine the location and indicate the degree of gout development.
- CT (computer tomography). This method will allow you to obtain a series of images of increased accuracy. With their help, you can determine the degree of joint deformation, as well as the exact localization of tophi. The method is effective even in the early stages of the disease.
- X-ray of the affected joints. Prescribed at early stages to determine the type of disease and exclude the presence of other joint diseases. In gout, X-ray images do not allow to capture changes. This method is effective only in the chronic form of the disease.
Differential diagnosis
Laboratory tests together with instrumental diagnostics allow to obtain a full picture of the disease. They are aimed at identifying the process of formation and excretion of uric acid at various levels. Differential diagnostics includes several areas: general blood and urine analysis, biochemical blood and urine analysis, examination of synovial fluid and tophi.
- General blood test. The study allows tracking the level of leukocytes. The analysis is carried out during an exacerbation, when an inflammatory process is observed. A change in ESR is possible, deviation from the norm of other indicators indicates the presence of severe kidney damage.
- Biochemical blood test. This method is the most important and significant. It allows you to determine the level of uric acid in the blood. Thanks to it, C-reactive protein indicators are monitored. Biochemical analysis indicates the presence of an increased level of creatinine, lipids and urea. It allows you to determine the quantitative content of prothrombin, fibrinogen and liver enzymes. These substances indicate the work of other organs and systems.
- General urine analysis. This method allows to detect changes indicating kidney damage. This condition is characterized by the presence of a large amount of urates in the urinary sediment. The presence of the albumin fraction of blood proteins is possible. This is due to damage to the epithelium of the renal pelvis by stones.
- Biochemical urine analysis. This method is necessary to determine the level of uric acid in the urine. It allows you to determine the amount of excretion of this substance during the day. Normally, it should not exceed 750 mg. If the kidneys are not affected, filtration occurs normally.
- A study of synovial fluid of joints. It is represented by a joint puncture, as a result of which it is possible to determine the level of leukocytes. The method allows to focus on neutrophils. This analysis is the most informative for making a diagnosis.
- Examination of the contents of tophi. When the growths are opened, they contain a large amount of crystalline powder. This symptom indicates the presence of gout.

Treatment gout on the feet
The main principle of treating the disease is to control the level of uric acid. To prescribe quality drug therapy, you need to seek help from a rheumatologist. Often, the treatment of gout on the legs involves the use of non-steroidal anti-inflammatory drugs. Usually prescribed: Metindol, Diclofenac and Indomethacin. To normalize the level of uric acid, they resort to the help of Allupol, Milurit and Allopurinol.
An important part of treatment is diet. It is necessary to maintain a certain balance between hunger and overeating. Rapid weight loss can lead to the production of large amounts of uric acid, leading to the development of gout. A similar effect occurs with overeating.
Sports activities play an important role. It is necessary to move more, do gymnastics, walk, run and ride a bike. Food should be varied, and most importantly, steamed. Salt is excluded completely, if it is impossible to follow this recommendation, its daily dose does not exceed a teaspoon. Spices and hot seasonings are removed without fail.
Following special rules will help eliminate attacks and ease a person's condition. To do this, it is enough to follow three recommendations: proper nutrition, medication, and sports.
How to relieve an attack of gout in the leg?
During attacks, it is necessary to provide the body with a sufficient amount of liquid. A special diet will help to cope with the chronic course of the disease. First, you need to practice increasing the amount of liquid consumed. It has been proven that 5-8 glasses of water per day can improve the condition and reduce the unpleasant consequences of an attack. It is important to avoid highly carbonated drinks. Products with a low purine content will effectively relieve an attack of gout, both on the leg and upper limbs.
It is necessary to eat more fresh fruits. They contain practically no purines, but they contain complex carbohydrates and nutrients that ensure good health. Fruits containing vitamin C will help prevent gout attacks. You should pay attention to oranges and tangerines.
It has been proven that proper nutrition helps relieve an attack of gout, and not the use of any compresses or other methods. The main task of a person is to reduce the amount of uric acid produced, and this can only be achieved with proper nutrition. The attending physician should give special recommendations on this matter.
It is imperative to exclude alcohol from the diet. This drink is characterized by a high content of purines and can worsen the situation. Moderate consumption of wine reduces the likelihood of developing gout.
Medicines
To eliminate the disease, drugs are widely used that are aimed at normalizing the level of uric acid and have an anti-inflammatory effect. Often prescribed drugs such as: Metindol, Diclofenac and Indomethacin. They have a pronounced anti-inflammatory effect. The following help stabilize the level of uric acid: Allupol, Milurit and Allopurinol.
- Metindol. The active substance of the drug is indomethacin. It is not recommended for use by people with hypersensitivity. The drug is used in the amount of 1-2 tablets per day. The attending physician can adjust the dose. The drug cannot be used during pregnancy, people with Crohn's disease and congenital heart defects. It can cause: diarrhea, erosion of the mucous membrane of the gastrointestinal tract, Quincke's edema.
- Diclofenac. The drug is used 1-2 tablets per day, depending on the patient's condition. The course of treatment does not exceed 6 days. The drug should not be used by people with gastric ulcer, urticaria and bronchial asthma. It can lead to the development of dyspeptic phenomena, drowsiness, irritability and headaches.
- Indomethacin. The drug is used 1-2 times a day, one tablet at a time. The drug has a pronounced anti-inflammatory effect, it should be taken according to the doctor's recommendations. It is prohibited to use it in case of gastrointestinal tract disorders and sensitivity to indomethacin. It can lead to the development of nausea, diarrhea and allergic reactions.
- Allupol. The drug is used after meals. The daily dose is 200-300 mg of the drug. If necessary, it is increased to 600 mg. The drug should not be used in case of liver failure, diabetes, acute gout attack and in children under 14 years of age. It can lead to the development of myalgia, dyspepsia, urticaria and skin itching.
- Milurit. The drug is a complete analogue of Allupol. It is taken in a similar dosage, 200-300 mg per day, with the possibility of increasing to 600 mg. Side effects and contraindications are identical to Allupol.
- Allopurinol. The dosage of the drug is prescribed individually. The dose can vary within 100-900 mg per day. It is not recommended to use the drug for people with impaired kidney and liver function. It can lead to the development of cardiovascular disorders, gastrointestinal tract and allergic reactions.
Ointment for gout on the feet
Drug treatment involves complex therapy. Taking medications orally may not be enough, so the treatment on the legs is supplemented with special ointments for gout. They are particularly effective and have an effect. Most often, they resort to the help of Diclofenac, Butadion, Ketoprofen and Fulflex. Previously, Vishnevsky ointment was widely used.
- Diclofenac. Apply the product to the affected area 2-3 times a day. The duration of use is discussed individually. The drug cannot be used with open wounds and hypersensitivity to diclofenac. May cause urticaria, itching and redness.
- Butadion. The product is applied in a thin layer to the affected area 2-3 times a day. The duration of use does not exceed 10 days. Do not use if the skin is damaged or if there are allergic reactions. May cause redness, itching and peeling at the site of application.
- Ketoprofen. The gel is applied in a thin layer to the affected area of the body 2-3 times a day. The duration of use is discussed with the attending physician. It cannot be used on open wounds, during pregnancy and for children under 6 years of age. It can lead to allergic dermatitis, eczema and skin rash.
- Fulflex. The drug is applied to the inflamed area of the body in a thin layer. 2-3 applications per day are enough to achieve a positive therapeutic effect. Do not use in case of hypersensitivity. No side effects were noted.
- Vishnevsky ointment. The product is applied to the affected area several times a day. The ointment has no contraindications or side effects.
Folk remedies
Traditional methods of treatment can help the body cope with many diseases. Over time, a huge number of different recipes have accumulated. Traditional treatment is aimed at a quick recovery of the body without causing harm to it.
- Recipe #1. Turnip applications. Take a turnip and boil it, add 2 tablespoons of vegetable oil if necessary. Cool the resulting mixture and apply to the affected area of skin. If tophus has formed on the joint, the application is prohibited.
- Recipe #2. Spruce cones are a great way to cope with an attack of gout. You need to take the ingredient in an unopened form and pour 1.5 liters of liquid over it. The tincture should stand overnight, after which it is used 3 times a day 30 minutes before meals. It is advisable to repeat the procedure until complete recovery.
- Recipe #3. Bay leaf will help cleanse the joint. Take 5 grams of the ingredient and pour 1.5 glasses of liquid over it. Let the remedy brew for 3 hours, then drink it within 24 hours.
- Recipe #4. Ordinary boiled rice has a cleansing effect. It needs to be washed thoroughly, poured into a jar and filled with water. The rice is soaked overnight and boiled in the morning. Then it is washed again and boiled again. The procedure needs to be repeated 4 times. The resulting "dish" should be eaten without salt and oil. After eating it, you cannot drink anything for 4 hours. The duration of treatment with this method is 45 days.
Mineral water for gout on the feet

Water alone will not be enough, so you need to follow a healthy diet. The maximum therapeutic effect is achieved by using medications.
Herbal treatment
Medicinal herbs can have a positive effect in the treatment of many diseases. Gout falls into this category. Herbal treatment allows you to achieve good results. You should not resort to it on your own, due to the fact that some plants are poisonous.
- Recipe #1. Chamomile decoction is great for soothing an inflamed limb. To prepare it, take 100 grams of the plant flowers and pour 10 liters of liquid over them. When the solution is infused, make a bath out of it. Repeat the procedure until a positive effect is observed.
- Recipe #2. Common mullein. Dry flowers of the plant should be poured with vodka or alcohol. The mixture should be infused for 7 hours. After the specified time, begin rubbing the product into the affected areas of the skin.
- Recipe #3. St. John's wort infusion. Take 2 tablespoons of the main ingredient and pour a liter of boiling water over it. Let the remedy steep for 3 hours. Then you can take 50-70 ml 3 times a day. In chronic cases, the treatment lasts several months.
- Recipe #4. Chicory. A couple of tablespoons of the main ingredient should be poured with 200 ml of boiling water. The remedy is infused for 5 hours. It should be taken half a glass no more than 3 times a day during meals.
Homeopathy
Gout is an acute disease characterized by severe attacks. It can be treated with both traditional medicine and homeopathy.
In severe attacks, ACONITE 30 is recommended. Chronic disease can be alleviated with AMMON. PHOS. 30. Reddened and inflamed joint of the big toe requires ARNICA 30. In typical gouty symptoms, BELLADONNA 30 is used. In the presence of severe swelling, BRYONIA 30 is used.
Sharp soreness and excruciating pains are eliminated with COLCHICUM 30. GUAIACUM 30 will help get rid of tophi. If the condition worsens, LEDUM 30 should be taken. Acute and chronic forms require the use of SABINA 30. URTICA 30 will help relieve a sudden attack.
Homeopathic medicines can be taken only with the permission of a doctor and under his supervision. Self-prescribing treatment is not recommended.
Surgical treatment
The most radical method of treatment is surgical removal. It is aimed at eliminating the growth - tophus. Surgical treatment is used in cases of severe gout. Surgical intervention involves removing uric acid from soft tissues.
Operations of this kind are mainly carried out in Israel or Germany. Medication only allows to stop the attack, but does not allow to get rid of the disease completely. Many clinics in Russia practice gout removal. The treatment is comprehensive, its cost does not exceed the permissible limits.
Before surgery, the human body is fully examined. It is necessary to know all the blood parameters and the level of uric acid in it. Then preparation for surgery is carried out. It should be understood that after the removal of tophus, it may appear again. Not immediately, but after a certain time. Gout is a lifelong disease. Treatment is aimed at maintaining the person's condition.
Diet for gout on the feet
When you have this disease, you need to follow a special diet, this will reduce the level of uric acid in the body. A diet for gout on the legs requires limiting the consumption of certain foods. So, you should almost completely exclude meat, poultry and fish. These foods are rich in purines. You should refuse fatty fish, meat by-products and canned fish. You should reduce your fat intake. They can affect the body, preventing the removal of uric acid from it. It is advisable to refuse asparagus, beans and cabbage. The diet involves limiting the consumption of alcoholic beverages. Alcoholic beverages provoke the development of gout. Their main effect is aimed at blocking the kidneys, which prevents the removal of uric acid. It is permissible to drink 250 ml of dry wine per day, but not more than 4 times a week.
Fructose is prohibited, so products based on it cannot be consumed. This list includes confectionery, ketchup, corn syrups and juice drinks. Complex carbohydrates should be preferred. You can eat whole grain products, fruits and vegetables. Confectionery products are prohibited, this includes white bread.
It is advisable to eat low-fat dairy products. This can be low-fat yogurt or milk. Finally, a large amount of liquid is the basis of any diet aimed at eliminating gout. Diet 6 for gout on the legs
In addition to excluding prohibited foods from the daily diet, it is necessary to adhere to a healthy diet. You should eat 5-6 times a day, in small portions, its volume should not exceed one glass. The main essence of diet 6, used for gout on the legs, is to limit daily calorie intake. Great importance is given to liquid, its amount should not be less than 2 liters per day. It is allowed to drink alkaline drinks, fruit drinks, rosehip decoction, tea and coffee.
Experts recommend fasting days. It is advisable to allocate 1-2 days a week for this. The essence of fasting is to eat one product throughout the day. It is necessary to give preference to apples, kefir, cucumbers or compote.
A diet for gout is the observance of a complete and healthy diet. It should be followed not only during attacks of the disease, but also by healthy people. It is necessary to eat a certain amount of proteins, fats and carbohydrates. The diet should be diluted with vegetables, fruits and vitamins.
The list of prohibited products includes any meat dishes, including broths. Mushrooms and fish are prohibited, these products are too heavy for the stomach. You will have to give up sausages and smoked meats. This also includes canned goods, salted fish and cheese. Daily salt intake should be minimal, seasonings should be completely abandoned. Those with a sweet tooth will have a hard time, because eating chocolate is also not recommended. If the patient is overweight, bakery and confectionery products should be abandoned.
The permitted list is not as meager as it may seem. So, 3-4 days a week you can eat lean meat and fish. You can treat yourself to chicken eggs, but no more than one per day. Meat must be boiled. It is worth taking a closer look at fermented milk products. Preference is given to cottage cheese, kefir and sour cream. The main diet should be fruits and vegetables. A bathhouse and sauna will help you cope with the disease. Before resorting to this method, you need to consult a doctor. A bathhouse cannot be called a panacea for all diseases, but still, there is some benefit from it. This method has long been used to alleviate the general condition. Traditional medicine and a bathhouse for gout on the legs are two fundamental components. They can be combined together by preparing decoctions from medicinal herbs and steaming your feet.
Contrast procedures are what you need to do in a bathhouse. You need to prepare herbal decoctions and alternately dip your feet in cold and hot water. It is recommended to massage the affected joints. In the bathhouse, instead of tea, you should drink rosehip decoctions or elderberry leaf tincture. This will enhance the positive effect. Depending on the course of the disease, a certain set of exercises is prescribed. With gout on the legs, the effect of gymnastics is aimed at alleviating the condition and maintaining joint mobility.
The simplest movements are bending and unbending the joints. This can be supplemented by rotating the foot in different directions. Often, salts are deposited in the big toes, an effective exercise will help "break" them.
