The term "frenulotomy" refers to any operation in which the tension of the bridle is surgically relaxed. However, most often this term is used when talking about the operation to weaken the frenulum of the foreskin. In turn, what is a "bridle"? We are talking about an anatomical element belonging to one or another organ and limiting its motor amplitude. In the human body there are several organs equipped with a similar element: these are the organs of the genital sphere (labia, clitoris, penis), as well as the tongue and lips. Therefore, it is not quite correct to call frenulotomy only an intervention on the bridle of the penis: it is a general term affecting all types of such operations.
Indications for the procedure
Frenulotomy in the form of surgical loosening of the frenulum in male patients (foreskin) is necessary if its length is not enough to perform certain functions. You can suspect that a person has a shortened bridle of the foreskin, in the presence of such symptoms:
- with the appearance of pain during sexual contact;
- with early ejaculation;
- with a shortened intercourse;
- with frequent injuries and bleeding of the frenulum;
- with the lower bend of the penis in the erection phase.
A large number of vessels and nerve fibers are combined in the bridle: during the movements of the foreskin, the nerve endings are mechanically irritated, which leads to increased arousal and a state of orgasm. If the bridle is injured during the act, it causes premature ejaculation, pain and discharge of blood from the damaged vessels.
Why is the bridle shortened? This can be a congenital abnormality, cicatricial post-traumatic change. Frenulotomy is indicated for such painful conditions as phimosis and paraphimosis.
In the oral cavity, too, there are several bridles, in relation to which sometimes it is necessary to resort to frenulotomy. For example, between the lips and gums there is a small element of the mucous membrane, which is also called a bridle. If this element is too short, then gingival inflammation may often develop, due to tissue retension and a gradual "tearing off" of the periodontal mucous layer from the maxillary alveolar process. Frequent inflammatory reactions eventually develop into gingivitis, periodontitis, with a further violation of the dentition.
In young children, a too-short bridle of the tongue can cause impaired speech, the formation of an abnormal bite, and even a disease of the digestive system (due to improper chewing of food).
In each of these situations, the doctor may advise you to do a simple operation - frenulotomy.
Preparation
In most cases, frenulotomy does not require any preparatory activities. This is a short-term and simple operation, which is performed under local anesthesia.
It is good if there is an opportunity to pre-select an experienced doctor who has already made such corrections and knows all the nuances of pathology.
In some cases, there is a need for a preliminary diagnosis: the doctor may advise you to take a complete blood and urine test, and also to exclude HIV, syphilis, and hepatitis.
If a bleeding disorder is suspected, a coagulogram will be prescribed.
Directly at the clinical institution where the frenulotomy intervention will be performed, the health worker will test for the absence of an allergy to the anesthetic substance.
All the above procedures are done, first of all, in order to determine whether the patient has contraindications to the operation.
In the morning before the frenulotomy procedure, you must take a shower. If the operation is performed in the oral cavity, then teeth should be thoroughly cleaned, and if in the genital area, they should be shaved and clean clothes should be put on.
Hospitalization after frenulotomii not required. As a rule, the patient is sent home a few hours after the procedure. After 2 weeks, you must visit a doctor for a check-up.

Technique of the frenulotomy
The frenulotomy procedure is carried out with the use of local anesthesia (only in rare cases, resort to general anesthesia, if there are indications).
The whole frenulotomy operation lasts from ten to thirty minutes, depending on the situation. The doctor performs the following actions:
- processes the transaction field;
- performs a cross-section of the dermal junction;
- stitches longitudinally.
If, during the examination, the doctor finds scars that may have remained after the injuries of the bridle received earlier, then he gently excises them with a scalpel.
When suturing the wound, self-absorbing material is more often used: it perfectly fixes the tissue and does not require subsequent removal of sutures.
After frenulotomii in the hospitalization of the patient is usually not necessary: the person rests for a while, and with satisfactory health goes home.
Types of frenulotomy
There are two types of operational frenulotomy - a classic look and laser plastic. Briefly consider each of them.
A conventional, classic frenulotomy is the most commonly practiced type of surgery, in which the doctor performs direct access with a scalpel. The surgeon cuts the skin element, thereby extending it. Advantages of this method:
- operation is available;
- Any practicing surgeon can do it.
The disadvantages of this frenulotomy method are:
- there is a risk of infection in the wound;
- the recovery period is accompanied by pain;
- the healing process takes longer;
- during and after surgery, bleeding may occur in the wound.
Frenulotomy is more easily perceived by patients by a laser, but the equipment for such an operation is not available in every hospital. The advantages of laser frenulotomy are obvious:
- minimum tissue damage;
- minimum risk of infection in the wound;
- minimal pain;
- lack of postoperative bleeding;
- the intervention itself is faster, and the rehabilitation period is significantly shortened.
The disadvantages of this method of treatment are that the penis is an organ that can change its size involuntarily, and the doctor does not have the ability to control this process. What does it affect? The fact that after laser frenulotomy for some time there will be a likelihood of microcracks and tissue breaks.
The frenulotomy of the foreskin is the only way to solve the problem with a short bridle. The standard plastic of the short frenulum of the foreskin consists in a transverse dissection of the skin element, with further overlapping of the longitudinal seams. The intervention takes place on an outpatient basis, without the need for further inpatient treatment. Another variant of plastics is also possible: the incision is made in the form of the letter V, and the seams are superimposed with the letter Y, which helps to make the bridle 10-15 mm longer. It should immediately be pointed out that the first standard version of plastic frenulotomy is preferred. In another embodiment, the risk of phimosis increases (due to the narrowing of the foreskin), and after the intervention an unaesthetic scar remains.
If a man often had gaps in the bridle, as a result of which multiple cicatricial changes formed on her, then he is recommended to remove this element. Such an operation already has another name: frenulectomy. This type of operation is carried out by a cross-section along the diametrical circle of the penis (or its part), which allows to reduce the tension of the prepuce. The tissue is sewn longitudinally: a scar is formed in place of the skin fold, which coincides with the natural suture running in the lower segment of the penis.
With phimosis, frenulectomy does not always lead to a positive result; therefore, most experts recommend surgical circumcision, better known by the people as “circumcision”.
Frenulotomy of the upper lip of the frenulum is also one of the variants of the operation, which is carried out to correct the bite and speech problems in children. In this case, the procedure is subjected to a fold of mucous tissue, responsible for attaching the lower and upper lips to the jaw bones. This intervention is also carried out under local anesthesia: cut through the bridle and connect it to the desired site.
Frenulotomy of the tongue is a no less common procedure in pediatrics. It has to be addressed if the baby’s sucking function is impaired: it’s difficult for the baby to suck the breast normally, his appetite is disturbed, he loses his weight. In this situation, the use of frenulotomy more than justified.
The shortened bridle of the tongue in medicine is called ankyloglossia - it is a congenital maxillofacial defect, which leads to a limitation of the motor ability of the tongue. In general, there are three bridles in the mouth in relation to which frenulotomy can be applied: this is the sublingual fold, as well as the frenulums of the upper and lower lips.
Contraindications to the procedure
Such surgery as frenulotomy will not be performed in the following situations:
- with active infectious processes in the body;
- if a patient has such diseases as hepatitis, HIV or venereal pathology;
- in cancer cases;
- with impaired blood clotting;
- inflammatory processes of the urogenital tract.
Some of the listed contraindications to frenulotomy are relative. Therefore, a potential patient should not draw independent conclusions about the possibility or impossibility of the intervention. Any doubt should be discussed with the doctor: frenulotomy, albeit simple, but still an operation in which there may be various unforeseen complications.
Consequences after the procedure
Frenulotomy among surgeons is considered a relatively simple intervention, in which unpleasant consequences occur quite rarely. However, each patient should be aware of the risks to which he is exposed when going to such an operation.
Undesirable consequences after frenulotomy may be:
- the addition of infection, the development of inflammation (balanoposthitis);
- seam divergence;
- bleeding;
- allergic response to agents used for local anesthesia.
Insufficient postoperative treatment of wounds after frenulotomy, weakening of immunity, infection from the urogenital tract can cause the development of an inflammatory reaction. Most often, this reaction takes place in the form of balanoposthitis - an inflammatory process affecting the head and foreskin of the penis.
Balanoposthitis, as a result of frenulotomy, manifests itself with such painful symptoms:
- redness of the skin in the head;
- swelling of tissues (swelling);
- pain sensations;
- odor;
- inflammation of the nearest lymph nodes.
If after frenulotomii appeared at least one of these symptoms, you need as soon as possible to visit a doctor. If you ignore the development of balanoposthitis, or try to cure the problem yourself, then there will be other, more serious complications:
- purulent inflammatory processes;
- inflammation of the urethra, bladder, etc.
Complications after the procedure
Any surgical intervention has a certain degree of probability of complications - for example, after operations there is a risk of bleeding or an infectious process. After frenulotomy, fortunately, there can be no life-threatening blood loss, because there is no large-vascular network in the bridle area, but only capillaries.
But with a wound infection, the situation is different: bacteria can actually get into the surgical wound, and both during and after the intervention (the latter occurs much more often).
In order to avoid inflammatory and infectious complications on the background of frenulotomy, it is necessary to follow all doctor's instructions regarding the care of the operated area in subtleties. In most cases, the doctor recommends systematically treating the wound with antiseptics, hydrogen peroxide, antibacterial ointments, etc.
Care after the procedure
Uncomfortable or painful sensations after frenulotomy are considered normal postoperative symptoms. Usually they last no more than a couple of days. The patient may experience particular pain at times of erection - for example, in the morning. To ease the pain, the patient is advised to walk around the room, take a few deep breaths, and distract. Sometimes, to improve the condition of the operated, the doctor prescribes special soothing medicines, analgesics. In addition, in most cases, the use of external antiseptics and oral antibacterial drugs.
During the first days after frenulotomy, a bandage should be applied to the penis. Immediate care is carried out using warm running water with detergent.
The stitches are removed after about 10 days (if no self-absorbable material has been used).
Complete healing of tissues takes about 20 days. After this time, you can resume sexual activity - but, as they say, without fanaticism, carefully. If the discomfort returns, you should wait a few more weeks. It is believed that the discomfort can theoretically persist for up to 4-8 weeks. If during this period, the pain did not pass, then you need to register with a doctor.
