Perfusion of the uterus depends on the reproductive age of the patient and the use of hormone replacement therapy. The normal picture of the uterine artery spectrum is characterized by a high blood flow velocity, an index of resistance greater than 0.5, and the presence of a postsystolic jug.
The value of the index of resistance of the uterine artery and vessels of the endometrium within 0.4-0.5 may reflect an increase in normal perfusion in the second half of the cycle or evidence of neoplasm of the uterus or endometrium. A resistance index of less than 0.4 allows one to suspect a malignant neoplasm.
Uterine fibroids are usually characterized by peripheral vascularization. Perfusion with myomas decreases in response to hormone replacement therapy. In this case, ultrasound dopplerography is used to monitor the response to treatment.
Hormone therapy in conditions of artificial insemination can significantly improve perfusion in the uterus. Some studies have shown that a ripple index of more than 3.0 is associated with suboptimal perfusion of the uterus and unsuccessful embryo implantation.
