Not all discharge in men indicates pathology and the development of serious diseases, there is also a physiological discharge of secretion from the penis, usually transparent. Unfortunately, not always diseases of this sphere are manifested by obvious symptoms, so white discharge should be alarming. A visit to a urologist will clarify the situation and identify the problem at the initial stage, which is important not only for the man, but also for his family or sexual partner.

Epidemiology

According to statistics, more than one million people are infected with sexually transmitted infections every day in the world. Thus, in 2016, WHO recorded 127 million cases of chlamydia, 87 million cases of gonorrhea, and 156 million cases of trichomoniasis. - Gonorrhea, 156mn - trichomoniasis. In the USA, prostatitis accounts for 8% of all urological diseases.

Causes of the white discharge in men

The appearance of white discharge in men has several causes:

  • Candidiasis (caused by the fungus Candida); [1]
  • mycoplasmosis (provoked by tiny bacteria - mycoplasmas); [2]
  • Chlamydia (chlamydia infiltrates the cell, multiplies, and destroys it); [3]
  • prostatitis (inflammation of the prostate gland); [4]
  • Gonorrhea (a sexually transmitted disease caused by gonococcus). [5]

Risk factors

Inappropriate intimate hygiene, promiscuous sexual relations, unprotected contacts contribute to the spread of sexual infections and sexually transmitted diseases.

Pathogenesis

Each of these diagnoses has its own pathogenesis. For example, in candidiasis, fungi attach to the walls of the epithelium, colonize and deeply infiltrate it. Their penetration into the body in 30-40% of cases contributes to sexual contact with an infected partner.

Mycoplasmosis, chlamydia, gonorrhea - with the different structure of pathogens (with or without cell membrane) they produce intracellular parasitization, eventually destroying the cell, causing inflammation.

Symptoms of the white discharge in men

White discharge on men's underpants is a fairly common phenomenon, which does not always indicate health problems. They accompany sexual excitement, their appearance in the morning indicates morning semen, which is quite normal for men of young age.

White discharge in men on the head without odor, not carrying discomfort in most cases are physiological in nature and should not cause concern.

The first signs of trouble are signaled by white discharge with an odor. This can be an alarm bell, indicating infection with gonorrhea. In addition, there is pain in the groin, burning during urination, fever.

Unpleasant odor also appears with mycoplasmosis, chlamydia. The latter is one of the most common sexually transmitted diseases. It is often asymptomatic for a long time, and then there is white vitreous discharge from the urethra in men and painful urination, itching, pain in the scrotum, lower back, testicles.

Mycoplasmosis gives similar symptoms: during the emptying of the bladder from the urethra are secreted white, they are also detected in the morning, urination is difficult and frequent, there is pain in the area of the scrotum, recoiling in the anus.

If white curdy discharge appears on the head of the penis in men, having a dense concentration and sour odor, and the process of urination is accompanied by burning, itching, soreness, then the diagnosis is predictable - candidiasis or thrush. The disease affects the head and foreskin, under which white flakes also accumulate.

For prostatitis in addition to white discharge is characterized by pronounced clinical manifestations in the form of fever, fever, chills, pain in the lower back, abdomen, anus, signs of general intoxication (general weakness, fatigue), pain during defecation and urination, rubbing, sexual dysfunction.

Complications and consequences

In most cases, these diagnoses are dangerous with consequences and complications. For example, acute inflammatory process in the prostate gland can lead to cystitis, pyelonephritis, general blood poisoning.

Bacterial sex infections often end in infertility. Gonorrhea is dangerous with the development of complications such as balanoposthitis, couperitis, paraurethritis, prostatitis, veseculitis, formation of narrowing of the urethra. Male sexual function may also be affected.

Diagnostics of the white discharge in men

Diagnosis of diseases accompanied by white discharge from the genitals consists of a number of examinations, including a doctor's examination, tests, instrumental examination. The patient will be sent to a general urine and blood test, for sugar, take a smear for culture, which allows you to identify the causative agent and its reaction to antibiotics. The material for bacterioscopic analysis is a secret from the prostate gland, a scraping from the urethra.

A comprehensive approach to detecting the disease involves computerized tomography, ultrasound, urography.

Differential diagnosis

Many diseases of the male genital sphere with white discharge have similar symptoms. The task of differential diagnosis is to determine the true cause of this phenomenon, to identify the causative agent and direct therapeutic measures to destroy it.

Treatment of the white discharge in men

The diseases listed above are treated mainly with antibacterial drugs that have a broad spectrum of action. If intracellular bacteria are detected, an antibiotic capable of penetrating into the cell is prescribed.

Fungal infections are treated with antifungal agents. Painkillers and non-steroidal anti-inflammatory drugs may also be used.

Both internal administration of medication and topical treatment are used.

In the treatment of gonorrhea, combined antibiotic therapy is used, consisting of these drugs: ceftriaxone intramuscularly and orally together with azithromycin or with doxycycline twice a day.

Ceftriaxone is a III generation antibiotic with prolonged action, powder for solution. It is injected once a day at 255mg. Can cause side effects in the form of hypersensitivity reactions, dyspnea, bronchospasm, headache, dizziness, diarrhea. Contraindicated during pregnancy and lactation.

Azithromycin is a broad-spectrum antibacterial agent. It is administered once in a dose of 1g. May cause nausea, vomiting, flatulence, stomach pain, diarrhea, general weakness, anxiety, convulsions, arrhythmia, tachycardia. It is not prescribed in case of hypersensitivity to its components

In the case of prostatitis, an antibiotic that is sensitive to the microorganisms that caused it is determined. This may be moxifloxacin, levofloxicin, minocycline, levomycetin.

Moxifloxacin is a drug from the quinolone group. It is administered intravenously once a day in the form of slow infusions of 400mg, lasting 60 minutes. After a few days, you can switch to oral administration of tablets. The course of treatment on average varies between one and two weeks. Side effects include allergies, anemia, a state of nervous excitement, dyspepsia, gastritis. Contraindicated in children under 18 years of age, pregnant, lactating women, people with serious disorders of cardiac activity, liver function.

Sexually transmitted bacterial infections such as mycoplasmosis, chlamydia are successfully treated mainly with erythromycin and its derivatives - macrolides, one of which is fromilide.

Fromilide is an acid-fast antibiotic and also has an immune-stimulating effect. It is taken one tablet twice a day for 7-14 days. The drug is safe to use (has less than 1% of side effects). It is especially effective when combined with cyprinol.

Candidiasis in men is treated mainly with topical agents such as Zalain cream, Lomaxin 2%.

Zalain is a synthetic antifungal drug. It is applied in a thin layer on a clean washed organ in the morning and evening. The course of treatment can be up to 4 weeks. Ointment is well tolerated, in rare cases there is itching, redness, peeling.

Any pathology is easier to overcome by strengthening the immune system. Vitamin and mineral complexes and foods with a balance of vitamins and minerals will help to strengthen the defenses. For men, they should include vitamins A, C, E, K1, B group, folic acid, biotin, iodine, calcium, magnesium, potassium, zinc, selenium, chromium and others.

Physiotherapeutic procedures are widely used in the complex therapy of prostatitis. The need for them arises because drugs do not penetrate well into the prostate gland, where their concentration is much weaker than in the blood serum. To help come such procedures as finger massage of the prostate gland, its electrical stimulation, magnetotherapy, rectal and urethral heat treatment.

For the treatment of chronic gonorrhea and its complications are widely used warming and medicinal compresses, mud treatment, the use of wraps ozokerit, paraffin, UHF, electrophoresis.

In other cases, physical methods are used to stimulate immunity processes, to carry out antiretroviral therapy.

Folk methods of treatment of white discharge in men involve external treatment of the head and foreskin of the penis, as well as ingestion of means that strengthen the body's resistance.

Most often herbs are used for this purpose. The most suitable for washing, applications of chamomile, calendula, St. John's wort, eucalyptus, oak bark, succession. It is best to prepare decoctions and infusions of their collections, which can also be taken internally. Boost immunity Echinacea, hemlock.

Homeopathy in no case will not replace drug treatment of white discharge in men, but will play a positive role in the complex therapy of diseases of the genital sphere, restore the balance of beneficial microflora. Among such drugs are Natrium muriaticum, Sulfum, Calium muriaticum, Sabal serrulata. Appointments, ways of taking and doses can only determine the homeopathic doctor.

All of these diagnoses are treated with conservative methods. Surgical intervention is used mainly when complications occur and normal urine flow needs to be restored.

Prevention

To prevent diagnoses in men accompanied by white discharge, you do not need much: adhere to a healthy lifestyle, observe intimate hygiene, avoid stress, fatigue, hypothermia, be choosy in sexual relations, use protective equipment in case of uncertainty about the safety of intimacy.

Macrolide antibiotics may be used as prophylaxis for genital chlamydial infection.

Forecast

Timely and competent treatment will rid the man of pathological discharge and will not give dangerous consequences.
