In the method of electrotherapy of patients with chronic prostatitis, doctors most often choose a more effective and safe microwave therapy in centimeter mode. The domestic device "Luch-2" is equipped with a ceramic contact rectal emitter with a special removable cover. It creates an electromagnetic field along the entire length, and the average depth of penetration of radiation into tissue is 4.6 cm. The nominal value of the fixed frequency of electromagnetic oscillations is 2 375 MHz. Microwave therapy increases the temperature in the prostate to 40 ° C, stimulates enzymatic processes, cellular and humoral immunity, activates natural killer cells, facilitating the removal of pathological secretion.
The effect of microwave microwave therapy on hemodynamics and vascularization of the prostate of patients with chronic prostatitis was monitored in 30 patients with chronic prostatitis. Before treatment, all patients complained of pain in various locations. Psychoneurological symptoms were present in 90% of patients. Discharge from the urethra was noted by 28%, general malaise - by 14%. Dysuria was observed in almost all patients, decreased potency - by 35%. Pathological changes in the analysis of prostate secretion were noted in 86% of patients in this group.
The patients received 8-12 microwave therapy procedures every other day, alternating them with massage. The patients were positioned on their right side during treatment, the exposure was 10-15 minutes.
As a result, 36% of patients stopped noticing psychoneurological disorders, and pain was relieved in 90%.
Dysuria disappeared in all patients. Sexual function was restored in 28% of patients. Prostatic secretion returned to normal in 86% of patients.
Control TRUS with Dopplerography performed after the course of treatment revealed an increase in the average value of peak linear velocity in the central zone of the prostate gland by 1.53 times, in the peripheral zone - by 1.24 times. The average value of diastolic linear velocity increased in the central zone by 1.25 times, in the peripheral zone - by 1.37 times. The average linear velocity in the central zone increased by 1.15 times, in the peripheral zone - by 1.5 times. No changes in the pulsation index and resistance index were noted. The increase in the average vessel diameter was more pronounced in the peripheral zone - by 1.29 times, in the central zone - by 1.15 times. The average value of volumetric blood flow velocity increased in the central zone by 1.63 times, in the peripheral zone - by 2 times. The density of the vascular plexus in the central zone increased by 1.7 times, in the peripheral zone - by 1.42 times.
Based on changes in the indices of prostate vascularization and its hemodynamics in a group of patients with chronic prostatitis, who received microwave therapy along with the generally accepted complex treatment, it can be said with confidence that this type of physical action has a beneficial effect on the prostate, promoting the restoration of its normal vascularization and hemodynamics in ischemic zones. The effect is more pronounced in the central zone of the prostate gland.
