Traditional treatment of prostate adenoma and urination disorders has been used for a long time. Currently, herbal preparations are most popular in Germany, Austria, Switzerland, France, Italy, Spain and Japan. According to modern concepts, the effectiveness of herbal medicines is ensured by the content of phytosterols, the most important of which are considered to be sitosterols. The following are considered as the supposed mechanisms of action of sitosterols in prostate adenoma (prostate gland):
- inhibition of the synthesis of prostaglandins (inflammatory mediators) in the prostate and providing an anti-inflammatory effect;
- decreased production of testosterone-binding globulin in the liver;
- cytotoxic effect on hyperplastic cells of the prostate gland:
- inhibitory effect on 5-a-reductase, aromatase and blocking effect on androgen receptors.
Thus, preparations based on Serenoa repens extracts have a complex antiandrogenic-antiproliferative effect on prostate adenoma (prostate gland). The complex anti-inflammatory effect of herbal preparations based on Serenoa repens is due to the ability of the extract to suppress the synthesis of prostaglandins (inflammatory mediators) by inhibiting the synthesis of phospholipase A2 , which is actively involved in the conversion of phospholipids into arachidonic acid, as well as inhibiting cyclooxygenase (reducing the production of prostaglandins) and lipoxygenase (reducing the production of leukotrienes). In addition, Serenoa repens extract has a pronounced anti-edematous effect, affecting the vascular phase of inflammation, capillary permeability and vascular stasis. Clinical results of the use of Serenoa repens extract indicate the achievement of a positive effect in most patients.
Permixon is an original drug of the French company Pierre Fabre Medicament, which has proven itself in many countries of the world for the treatment of adenoma (benign hyperplasia) of the prostate and chronic abacterial prostatitis. Permixon is an n-hexane lipid-sterol extract of the fruits of the American dwarf palm Serenoa repens, which has a non-competitive inhibitory effect on 5-a-reductase types I and II, local antiproliferative and anti-inflammatory effects. Unlike other drugs created on the basis of the same plant raw materials, Permixon is characterized by a manufacturing process certified by the US Food and Drug Administration (FDA) and the highest 5-a-reductase inhibition index.
The effectiveness of Permixon in patients with prostate adenoma is confirmed by evidence-based medicine data (category of evidence 1A) and is manifested by a significant reduction in obstructive and irritative symptoms of prostate adenoma (prostate gland), improved urination and a decrease in the volume of residual urine. Comparative studies have shown that Permixon is equivalent in effectiveness to the 5-a-reductase inhibitor finasteride and the most widely used alpha-blocker tamsulosin, while treatment with it is accompanied by a lower frequency of side effects. It has been proven that with long-term use (5 years), Permixon significantly reduces the volume of the prostate and the volume of residual urine, and completely controls the symptoms of prostate adenoma (prostate gland). It is characterized by excellent tolerability and safety.
Treatment of prostate adenoma with Serenoa repens extract is not associated with serious side effects. The effect of Serenoa repens extract therapy on sexual function has not been registered. Serenoa repens extract does not affect the hypothalamic-pituitary system, does not have estrogenic or antiestrogenic effects, does not interact with endogenous estrogens and is devoid of progesterone effects. In addition, the drug does not affect PSA levels.
Serenoa repens-based preparations do not change blood pressure, therefore they do not have a hypotensive effect and have no restrictions when used in people whose activities involve operating precision mechanisms, etc. The existing experience of long-term use of Serenoa repens preparations (over 5 years) has not revealed any significant side effects, this fact once again confirms the high safety profile of this preparation during long-term therapy.
The recommended duration of treatment for prostate adenoma with preparations based on Serenoa repens extract is 3 months or more.
Prostamol Uno is a herbal preparation based on the extract of Sabal palm (Serenoa repens) fruits. It has anti-inflammatory, antiexudative, and antiandrogenic effects. The antiandrogenic effect is achieved by inhibiting 5-a-reductase (types I and II) and aromatase, blocking the fixation of dihydrotestosterone to cytosolic receptors, which prevents the hormone from penetrating into the nuclei of prostate tissue cells and leads to a decrease in the synthesis of dihydrotestosterone. The anti-inflammatory and antiexudative effects are achieved by inhibiting arachidonic acid cascade enzymes: phospholipase A1, cyclooxygenase, and 5-lipoxygenase, which in turn leads to a decrease in the production of inflammatory enzymes (prostaglandins and leukotrienes).
Prostamol Uno has a high level of safety: it does not change blood pressure, does not affect the level of sex hormones and PSA in the blood, and does not change sexual function.
Contraindications
Individual intolerance to the drug and its components.
Method of administration and dosage: 1 capsule - 1 time per day, without chewing. with a small amount of liquid after meals. The recommended continuous course of treatment is at least 3 months.
Lipidosterol extract of Pygeum africanum tree bark is widely used in clinical practice. The active substance of this complex drug has not been isolated. The mechanism of action has not been fully studied either. During studies of the effect of Pygeum africanum extract on prostate growth factors, it was found that this agent, having virtually no effect on spontaneous proliferation of fibroblasts, inhibits their pathological growth. As a result of large-scale international studies on the effectiveness of Pygeum africanum extract in the treatment of prostate adenoma, significant clinical improvement was found in most patients with a minimum number (1.1%) of side effects.
The literature on the clinical use of other herbal medicines is relatively sparse, fragmented, and mostly without placebo control.
Thus, the analysis of data on the use of phytotherapeutic agents in the treatment of prostate adenoma allows us to state that the indicated drugs can have a positive effect on the subjective and objective manifestations of the disease. Herbal medicines can be widely used in urological practice as preventive agents in patients with uncomplicated prostate adenoma as an alternative to long-term dynamic observation. More large-scale studies are needed to objectively assess the effectiveness and determine the place of each of the phytotherapeutic agents and their combinations in the structure of prostate adenoma treatment.
The use of amino acid complexes for the treatment of patients with prostate adenoma currently has no scientific basis and is based on empirical data.
There is evidence that bovine genital extracts improve detrusor contractility, reduce sphincter concentration and improve their interaction and increase Qmax. These data need to be carefully verified.
Some of the safest drugs for the treatment of prostate diseases are bioregulatory peptides (prostatilen), the effectiveness of which is confirmed by numerous experimental and clinical studies.
The use of Prostatilen in a daily dose of 100 mg (Vitaprost Forte) in patients with prostate adenoma improves urination parameters, reduces obstructive and irritative symptoms, and significantly improves the quality of life. The etiopathogenetic mechanism of action of Vitaprost Forte on prostate adenoma is manifested in the preservation and development of the therapeutic effect after the end of the course of treatment.
It is important to note that 75-95% of patients with prostate adenoma have concomitant chronic prostatitis, which aggravates the clinical course of the underlying disease. Vitaprost Forte helps reduce the activity of the concomitant inflammatory process in the prostate and thereby increases the effectiveness of the treatment.
Vitaprost Forte does not change blood pressure, sex hormone levels, and maintains potency and libido.
Indications for use: prostate adenoma, conditions before and after prostate surgery, chronic prostatitis.
Release form Vitaprost Forte: rectal suppositories No. 10.
Method of administration and dosage: rectally, 1 suppository once a day. The minimum duration of treatment with Vitaprost Forte for benign prostatic hyperplasia is 15 days, for chronic prostatitis - 10 days.
Thus, drug folk treatment of prostate adenoma is an important element in the structure of treatment of this disease, it can be used in patients with clinically significant symptoms of the disease with minor or moderate severity of infravesical obstruction of the dynamic type. At the same time, despite the variety of drugs used to treat prostate adenoma at present, the criteria for selecting patients for drug treatment of prostate adenoma have not yet been sufficiently developed, there are no clear indications for the appointment of a particular drug in a specific clinical situation. This problem is one of the most important in the treatment of prostate adenoma and should be the subject of special scientific research.
