Upper eyelid plastic surgery in women differs significantly from that in men. In men, the surgeon must be extremely conservative. Usually, the lateral edge of the incision should not extend beyond the attachment of the lateral canthus. If the incision is made beyond the lateral edge of the orbit, the resulting scar cannot be covered with makeup. The surgeon must always remember that men do not use any makeup. In the long term, a scar, even a very light one, can remain visible for years.

The surgeon's tasks regarding the eyelid crease are very different in men and women. A deep upper eyelid crease is contraindicated in men. It not only feminizes, but also radically changes the appearance of a man's face. Men do not look like that. A man in modern society usually looks best with some excess upper eyelid skin, somewhat corrugated skin, and an eyelid crease located just a few millimeters above the lid margin.
