The main indication for liposuction is the presence of local forms of obesity with a violation of the contours of the figure. The most common deformation of the thighs in women is the breeches-like deformation of the thighs, which is created by fat "traps" located on the outer surface of the thighs. Less significant in size, but no less important in their influence on the line of the thigh are fat "traps" located on the inner surface of the knee joint. These "traps" are usually combined with fat deposits located in the area of the thigh, pelvis, abdomen and flanks.

The main goal of the operation performed for local forms of obesity is to correct the contours of the figure. At the same time, when discussing the possible results of the operation with the patient, the surgeon should emphasize that the main task of the intervention is not to create an ideal figure, but to achieve a noticeable improvement in comparison with the initial state.

In patients with significantly increased body weight, it is necessary to explain separately that liposuction is aimed specifically at correcting the contours of the figure, and not at reducing body weight. The latter is achieved only to a certain extent, but is rather a logical consequence of the removal of a huge number of adipocytes than the purpose of the operation.

In the case of generalized obesity and ineffectiveness of conservative treatment methods, single-stage or serial liposuction can give good results. The peculiarities are, firstly, that surgical treatment is aimed not only (and often not so much) at improving body contours, but also at reducing the patient's body weight. Secondly, this is achieved only by combining surgery with conservative treatment, the effectiveness of which increases due to the removal of a large number of fat cells during the intervention. Thirdly, the results of combined treatment depend on many factors and are less predictable compared to the treatment of fat "traps" in people with normal body weight. Accordingly, patients are often dissatisfied with the results of the surgery, which should be taken into account when preparing them for liposuction.

Good results can be achieved in the treatment of lipomas of various localizations and having a relatively low density.

For additional contour correction, liposuction can be used when performing other plastic surgeries. Thus, in abdominoplasty, it allows for a reduction in the thickness of the fat layer of the anterior abdominal wall, increased mobilization of flaps by tunneling their base with cannulas, and simultaneous removal of fat in adjacent areas. In rejuvenating facial surgeries, additional correction of areas of fatty hypertrophy located outside the skin detachment zone is possible. Most often, this is necessary in the mandibular, submandibular, and zygomatic areas.

In case of fatty and mixed forms of breast hypertrophy, performing reduction mastopexy in combination with liposuction allows for a more effective influence on shape and volume, as well as achieving greater symmetry.

Performing a mastectomy in combination with liposuction in the treatment of gynecomastia allows for resection of the mammary gland through a relatively small paraareolar incision, as well as the formation of the most natural transitional contour.

One of the problems of reconstructive surgery is the excess volume of transplanted skin-fascial (muscle) flaps due to subcutaneous fat. In this case, 6 months after the transplant, liposuction of the flap can be performed, which is an effective method for correcting its contours.

The operation is performed under local anesthesia, and in the postoperative period for 6-8 weeks the treated area is bandaged with an elastic bandage.
