Hair loss is not only a cosmetic problem but also a medical one. In both women and men, it is often associated with androgenetic alopecia, diffuse telogen effluvium following stress, illness, or deficiencies, and sometimes with cicatricial forms, in which the follicle is irreversibly destroyed. In this case, pills are perceived as a "quick fix," but in modern dermatology, systemic agents are considered only as part of a comprehensive treatment plan, not a magic pill. [1]

Systemic medications can be roughly divided into three broad groups. The first are vitamin-mineral complexes and nutraceuticals that replenish deficiencies of iron, vitamin D, zinc, biotin, and amino acids that influence hair growth. The second are specific medications with proven effectiveness for androgenetic alopecia, such as low-dose oral minoxidil and antiandrogen medications. The third are medications that are not considered "hair pills" but treat the underlying cause: thyroid medications, iron, vitamin D, and treatments for chronic diseases. [2]

It's important to understand: unless a doctor has determined the exact type of alopecia and assessed your overall health, prescribing any pills "by eye" will be more of a gamble than a cure. In cases of cicatricial alopecia, severe autoimmune processes, and active inflammatory scalp diseases, vitamins or "beauty" nutraceuticals alone will not solve the problem. In these cases, specialized treatment by a dermatologist or trichologist remains key. [3]

For androgenetic alopecia in both women and men, oral medications can significantly improve the prognosis, but only if started early and with the right combination of agents. A review of current approaches emphasizes that oral minoxidil, antiandrogens, and nutraceuticals work best in combination rather than individually. Furthermore, any systemic therapy should be complemented by topical treatments, gentle care, stress management, and correction of deficiencies. [4]

The tablets' specific purpose is not only to stimulate new hair growth but also to stabilize the situation: to reduce daily hair loss, improve the thickness and quality of the hair shaft, and reduce breakage. For the patient, this is subjectively perceived as "more hair," although from a trichoscopy perspective, it may represent a rather modest increase in density. Therefore, establishing correct expectations and discussing treatment goals before starting treatment are essential. [5]

Why does hair fall out and when are pills really needed?

Alopecia areata is an autoimmune disease in which the immune system attacks the follicle, forming round patches of hair loss. Treatment options for this condition are entirely different: immunosuppressive therapy, corticosteroids, targeted drugs, sometimes combined with nutraceuticals. Conventional "hair vitamins" and even specific antiandrogens play only a supporting role in this case. [8]

Scarring alopecia is the most dangerous form, in which the follicles are completely destroyed and replaced by connective tissue. In this case, any "hair growth" pills are useless unless the active inflammatory process is stopped. What types of hair loss pills are there: groups and mode of action

Vitamin and mineral hair complexes are the most well-known group of products. They contain biotin, B vitamins, vitamin D, iron, zinc, copper, amino acids, and sometimes plant extracts and antioxidants. Their purpose is to provide the follicle with everything necessary for keratin synthesis, normal matrix function, and adequate blood supply. In cases of true deficiencies, such complexes act as a treatment, and in cases of normal hair growth, as a gentle support. [11]

Nutraceuticals for hair are a separate class of "extended" complexes that, in addition to basic vitamins and minerals, contain specific components: marine protein extracts, polyphenols, probiotics, plant extracts with possible antiandrogenic effects, and amino acid complexes. Reviews show that such supplements provide a moderate but statistically significant improvement in hair density and thickness in women and men with androgenetic alopecia, especially when combined with standard therapy. A separate subgroup of nutraceuticals are supplements based on plant oils with possible antiandrogenic effects. Vitamins and Nutraceuticals: When They Really Help

Choosing the right systemic therapy begins not in the pharmacy, but in the doctor's office. During the initial consultation, a dermatologist or trichologist will collect a detailed medical history: when hair loss began, what illnesses the patient has been experiencing, what medications the patient is taking, and whether there are any accompanying symptoms (fatigue, shortness of breath, cold intolerance, menstrual irregularities). Then, an examination, trichoscopy, and basic blood tests are performed, including ferritin, vitamin D, thyroid function, and, if necessary, sex hormones. [26]

For androgenetic alopecia in women, the choice of pills most often comes down to a combination of oral minoxidil, an antiandrogen if necessary, and nutraceuticals. For telogen effluvium, the primary focus is on eliminating the trigger and correcting deficiencies, while specific growth stimulants are used as needed. [27]

The third step is assessing contraindications to systemic therapy. Before prescribing oral minoxidil, the physician assesses cardiovascular risk, edema susceptibility, and the presence of arterial hypotension. For women taking antiandrogens, pregnancy plans, the risk of thrombosis, the possible impact on the menstrual cycle, and the need for reliable contraception are discussed. In this setting, tablets are more effective and safer, and for some patients, especially those with telogen effluvium, proper lifestyle and deficiencies adjustments make it possible to get by with minimal doses of systemic medications. Security and Control: What Risks Are Important to Consider?

Despite their reputation as "gentle" remedies, vitamin-mineral complexes and nutraceuticals are not without risks. Excessive intake of vitamin A and vitamin D can lead to toxic effects, including increased hair loss, nausea, bone pain, and liver dysfunction. Excess iron is dangerous for patients with certain genetic iron metabolism disorders and liver disease. Therefore, high doses of certain vitamins and microelements are only permissible after laboratory confirmation of deficiency. [31]

Low-dose oral minoxidil has a favorable tolerability profile but is not without side effects. Mild swelling of the lower legs, increased heart rate, mild hypotension, and unwanted hair growth on the face and body are frequently reported. Research shows that combining oral or topical minoxidil with antiandrogen therapy, nutraceuticals, and gentle stimulation methods (microneedling, light therapy) produces more pronounced and stable results than any single approach. Each component of the regimen has its own specific purpose: tablets work internally, topical agents target the follicle, and ancillary treatments improve microcirculation and drug penetration. [36]

Nutraceuticals and vitamin-mineral complexes work best as a supportive supplement for "bigger" products. They create optimal conditions for hair growth, reducing the impact of deficiencies and gently supporting keratin metabolism. Topical treatments (minoxidil foams and solutions, growth stimulants, medicated shampoos, and serums) remain an important part of therapy even when switching to oral minoxidil. However, most reviews emphasize that a combination approach (a combination of systemic and topical therapy) provides a more stable and pronounced effect, and also allows for flexibility in reducing dosages. Does taking vitamins "just in case" for hair loss make sense?
If there are no laboratory deficiencies, taking high-dose vitamins "just in case" is not only useless but also potentially harmful due to the risk of hypervitaminosis. A more sensible approach is to get tested, choose a targeted treatment for any identified deficiencies, and, if necessary, supplement with a moderate-dose, complex nutraceutical. [42]

What should I do if hair loss doesn't improve with pills?
